Guide to Filling Out This Form
- Fill in all course and professor information requested.
- List the title and author of each article
- Provide complete citations for print journals (volume, page numbers, publisher, date)
- Provide the stable URL for articles from electronic databases, along with the name of the database, and date retrieved.

Please provide complete citations, in the order in which readings will be assigned. (In the event of a processing backlog, the reading assignment order will help us ensure that materials are ready when students need them during the course.)
Articles Please give as much of the following information as you have available for each article requested: author, publication date, title of article, title of journal, volume number, issue number, and page numbers. Please number each article request.
