In order for your professors to be notified of your
accommodations, you must request your Letter of Accommodation each
semester/term. In an effort to streamline the process and ensure a
timely response, students must now use this form to request their
letter. You must submit this completed form in order for your
request to be processed. You are encouraged to submit your request
at least one week prior to the beginning of the semester/term.
Please allow 48 business hours for your request to be processed.
You will be notified through your Saint Leo email when your request
has been processed and your letter has been sent. Any questions or
concerns should be directed to the Office of Disability Services at

if you prefer to request your letter in person.
