Residence Life Forms and Important Dates

To begin the process of housing assignments, the Office of Residence Life requires some very important forms from incoming students. Please review the letter from Residence Life and then download and complete your Residence Hall Agreement document and your Emergency Contact Form.

  • All University Campus students wishing to live on campus in the 2012-2013 academic year must complete & submit the following form: Residence Hall Housing Agreement (PDF)
  • All residential students who do not have an emergency contact on file must complete & submit the following form: Emergency Contact Form (PDF)
  • University Campus Students wishing to live off-campus must complete & submit this form for approval (must meet requirements): Request for Housing Termination (DOC)
  • If all residents in a room/apartment are 21 years of age (or older) they may apply for a "Wet Room".
  • Students wishing to appeal a decision by their Administrative Hearing Officer must complete this form, Appeal Form (DOC), and submit to their Administrative Hearing Officer by 5:00pm the next business day.
  • Students living in the Apartments 1-4 may select the 5, 10 or Unlimited meal plan and East Campus residents may select the 5, 10, Unlimited or no meal plan. All other areas require the Unlimited Meal plan. Students who are eligible for a meal plan change may request a change by competing & submitting the following form: Meal Plan Change Request Form.
  • Students who would like to request either a room or roommate change should complete the following form and submit it to the Office of Residence Life.
