Each student attending our traditional program at University
Campus full time is required to carry health insurance. This policy
applies to commuter students registered full time as well.
To assist students in following this policy, the University
automatically enrolls students in the University-endorsed health
insurance plan and bills their student
accounts. The insurance premium for the full
academic year is $1,046 and provides coverage from August 1, 2013 -
July 31, 2014. The insurance premium for the Spring-Summer
portion of the academic year only is $620. This provides coverage
for January 1, 2014 - July 31, 2014.
Students with their own insurance through a group or individual
plan may waive the coverage and avoid the fee. To waive the
insurance, students should complete the online waiver process.
Insurance waivers can only be submitted online. Insurance waivers
for students enrolling in Fall 2013 must be completed no later than
September 10, 2013. Insurance waivers for students enrolling in
Spring 2014 must be completed no later than January 25, 2014.
Please note insurance waivers are NOT guaranteed and may be
denied for various reasons. Reasons for denial include but are not
limited to the following:

unless they have coverage with a U.S. provider.
The deadline to submit completed waivers for students beginning
in the fall semester is midnight of September 10, 2013. The
deadline to submit completed waivers for students beginning in the
spring semester is midnight of January 25, 2014. There are no
exceptions to these deadlines.
Please note that students will not have access to the waiver or
enrollment forms until they are registered full time. When
accessing the waiver you will need to provide your 7-digit student
ID number and your date of birth.

If you plan to take the student health insurance plan, please

click on th elink marked 'Enrollment Form' to access the enrollment
form. IMPORTANT: You will not be able to access your insurance card
or your benefits until you complete the enrollment form. We
encourage you to complete the enrollment form at the carrier's
site early so that you can have access to your insurance
card and benefits as soon as possible after coverage starts on
August 1, 2013 (fall starts) or January 1, 2014 (spring starts). If
you use your health insurance benefits once coverage begins, you
are no longer eligible to waive coverage even if you submit a
waiver prior to the waiver deadline of September 10, 2013 (fall
starts) or January 25, 2014 (spring starts).
